How to Use This Template
- Fill in your company's actual leave entitlements per category — this template provides the structure, not fixed numbers, since entitlements vary by state and company policy
- Add a section on the sandwich leave rule if your company applies it (see our sandwich leave policy guide for how that calculation works)
- Set the effective date and a review date, and actually revisit it then — leave rules have changed with the 2026 OSH Code accrual standardisation, and policies written before that update may be out of date
- Circulate the finalized policy to all employees and keep a signed acknowledgement on file
